Puppy Application Questionnaire

Our Puppy Questionnaire is the first step in getting to know you! I only accept a limited number of families each year, so any and all information you would like to share to help us in our decision to see if we are a good match is greatly appreciated.

To be on our waitlist:

1. Fill out Puppy Questionnaire to be approved. (It helps us know you better)

2. Once we receive your deposit you will be added the list.

3. Read about Our Puppy Program "About our Puppies"

We realize the process may seem long, but we find the more effort being made to ensure our puppies will be living with a dedicated family the better success we all have. We will reply to your Questionnaire at our earliest convenience. Thank you for your interest in our Labs!

How did you hear about us? Would you mind sharing your instagram handle? (We like to follow along)

Are you applying for a puppy we currently have available or a future puppy? How soon are you hoping for a puppy?

Why did you decide on a Labrador Retriever for your new pet, and have you familiarized yourself with the characteristics of the Lab?

Do you currently own other dogs? How many and what breed?

Where will your puppy sleep at night? Will the puppy reside indoors or out?

If your backyard is not completely fenced in, how do you plan on containing your puppy/dog while he's outside?

Do you have a preference for color or sex of your puppy? Do you have a 2nd choice?

How how own

During the Puppy Stage will someone be home to supervise the puppy? How long will the puppy be on his/her own on average during the day?

Do you consider yourself an active person? Tell me about yourself. Puppies have energy!

What is your preferred method of training and Collars? Ex. Obedience Classes, Prong Collar, E Collar or Positive Praise? Please explain.

Have you ever given a pet away? If so, what were the circumstances?

Are you planning on crate training your puppy?

Are you willing to enroll in an organized obedience training with your puppy to help create a well behaved and sociable companion? If you are not, how do you plan on training your Dog? Please explain.

Is it your commitment to spay/neuter your Sunny Creek puppy? If you are interested in a female, we ask families to go through one heat cycle. Our Puppies/Dogs can not be Spayed or Neutered before 12 months of age. We feel it's important for Dogs to completely develop before being altered.

Are you willing to provide us with feedback at regular intervals as far as the puppy's social/physical development and how the puppy is adjusting to his/her new home? We would like to receive pictures and hear from you!

Do you understand that even though we have taken the time and expense to engage in extensive health screening before mating our Labradors there can be no guarantee that the puppy/dog purchased is completely free of genetic defect?

Our Puppies are priced at $4000.00 (Includes Sales Tax) Limited Registration (No breeding rights). If approved, are you wanting to place a $500.00 deposit? Please take a minute and read our Puppy Contract located under "About our Puppies"?
